IHSS Explained: What LA County Families Need to Know

When a parent starts needing more help with everyday tasks, families often find themselves asking the same question: What support is actually available?

For many Los Angeles County families, IHSS can help make it possible for an older adult or person with a disability to remain safely at home. But understanding who qualifies, what services are covered, and how to apply can feel overwhelming.

What Is IHSS?

In-Home Supportive Services, or IHSS, is a California program designed to help eligible older adults, people who are blind, and people with disabilities remain safely in their own homes instead of moving into out-of-home care.

For most recipients, IHSS operates through Medi-Cal and is funded through a combination of federal, state, and county dollars. Los Angeles County's Department of Public Social Services, or DPSS, manages applications and assessments locally.

The important distinction is that IHSS is not a home care agency. Once approved, the recipient generally hires and supervises the person who provides the authorized care. That provider may even be a family member, friend, neighbor, or someone found through LA County's provider registry.

What Does IHSS Cover?

The exact services depend on the person's needs and what the county authorizes after an assessment.

IHSS may help with services such as:

• Bathing, dressing and grooming
• Meal preparation and cleanup
• Laundry and basic housework
• Grocery shopping and errands
• Mobility and transferring
• Accompaniment to certain medical appointments
• Some paramedical services
• Protective supervision when eligibility requirements are met

The county determines both which services are approved and how many hours are authorized.

IHSS does not mean unlimited or automatic 24-hour care. It also does not simply pay for every household need. Services must fall within the IHSS program and be authorized based on the individual's assessed needs.

Who Qualifies for IHSS in California?

In Los Angeles County, an applicant generally must:

• Be 65 or older, blind, or have a qualifying disability
• Be a California resident
• Live in their own home rather than a hospital, nursing home, or licensed care facility
• Complete a Medi-Cal eligibility determination
• Participate in an in-home assessment
• Have a licensed healthcare professional complete a Health Care Certification showing that assistance is needed to remain safely at home

Some people with income above the no-share-of-cost Medi-Cal level may still qualify, although a Share of Cost may apply.

How to Apply for IHSS in Los Angeles County

1. Start the application.

2. Apply by mail or fax if preferred.

Complete the SOC 295 Application for IHSS.

3. Complete the home assessment.

A county social worker will interview the applicant and assess what tasks they can safely perform independently and where assistance is needed.

4. Complete the medical certification.

A physician or other licensed healthcare professional must complete the SOC 873 Health Care Certification before services can be authorized.

5. Wait for the Notice of Action.

The county will notify you whether the application was approved or denied. If approved, the notice will also explain the services and monthly hours authorized.

How Long Does IHSS Approval Take?

There isn't one guaranteed approval timeline published for every LA County case.

Think of it as a process rather than a single deadline: application, county follow-up, home assessment, Medi-Cal determination when needed, medical certification, and finally the approval decision.

While you're waiting, start documenting what your parent needs help with each day. Write down difficulties with bathing, dressing, meals, mobility, medications, household tasks, or staying safely alone.

That information can make the home assessment conversation much clearer.

A parent may need help with meals, bathing, mobility, supervision, or everyday routines before IHSS approval is complete.
